My mom is currently a patient here (about week 5), so this is based on ongoing experience. I will update after her discharge. She is 79 with limited mobility, dementia, anxiety, and was already on pain management prior to admission. After breaking her arm in four places and having surgery, she was transferred here for rehab. Because she only has use of one arm and limited mobility, she is fully dependent on staff for nearly everything. My sister and I are very involved and have had three care meetings with staff. The meetings are always positive, and we’re told things will improve—but then we hear a different story directly from our mom. What they do well: Rehab: This has been solid so far and is the main reason we chose this facility. Progress here is important, and they seem competent in this area. Cleanliness: I have visited at all hours and have not experienced the odor issues others have mentioned. Rooms are small and can feel stuffy, but they are cleaned regularly. Staff demeanor: Most staff are friendly and professional, with a few exceptions. Generally, they are pleasant when approached. Where there are serious concerns: Medication management: This has been the biggest issue. My mom was on multiple essential medications before arriving, yet there has been repeated confusion and poor communication. We recently discovered that her dementia medication had not been administered at all since admission without anyone informing us. That is unacceptable. Someone at this facility should definitely be in charge of reviewing all meds with family members. My mom has active and legitimate prescriptions. Pain management has also been extremely problematic. After surgery, she waited nearly eight hours for pain medication. Initially, meds were “as needed,” which requires her to request them (unrealistic given her cognitive challenges and the fact that call buttons are not answered promptly). We pushed for a scheduled approach, but it has been inconsistently applied and repeatedly changed. This has required constant follow-up from our family. Nighttime medications are also delayed. My mom typically goes to bed around 7 p.m., yet she often does not receive her meds until 9–10 p.m., which is not reasonable. Response times / call button: This is another major issue. My mom cannot get out of bed or use the restroom independently. There have been multiple instances where her call button went unanswered for extended periods, including one incident where she had an accident while waiting. This directly undermines her care and rehabilitation progress. One of the nurses or aides, actually told her “You can keep pressing it, we still aren’t coming any faster”. I’m sorry. What? How is that acceptable in any environment and how is there no standard response time. What if a patient has fallen or gotten really ill? That delayed response really could be life or death. This is a rehab facility serving primarily elderly patients with mobility and cognitive challenges. Timely response and reliable medication management should be basic standards of care. My mom is not an easy patient—we’ve been upfront about that. But two things are non-negotiable: proper medication management and timely assistance when she asks for help. We have limited options here in Lancaster. And I did a deep dive of all the facilities before we chose Buckeye. So we aren’t expecting perfection. But. We do expect kind, compassionate, and competent management of my mothers rehab.

Buckeye took excellent care of my father while he was there for rehab. He went from not being able to hardly walk after a hospitalization, to being able to walk with his walker and even climb stairs.

my grandma was here for a month & they took amazing care of her. very clean facility, amazing food, & great care team! she enjoyed the activities my husband Hunter put together for all the residents & one of the stna’s Gracie did an amazing job caring for her! would definitely recommend this facility for short term & even long term care! 🙂

My mother came in for rehab after experiencing severe issues with her back. Mother was treated great, ate well, and absolutely loved the activities. The stay was amazing for my mother.

My mom just spent a month there on the short term rehab unit. She absolutely loved it there. The staff were great with her. The food was good. Clean facility. She was sad to leave. She would definitely go back .

If you want your family member to smell piss and poop don’t bring here . When signing documents before admissions you won’t smell anything until your in the facility

My husband’s cousin was here for 3 weeks. They had him in with 3 other men and the room stunk of urine every time I visited. The cousin did not get dressed or out of bed the whole time he was there. We moved him to a different facility, and he’s getting dressed, walking around, and the place smells clean. Only 2 to a room and $100 a day cheaper!

My mom was a patient at Buckeye Care and Rehab after a complicated neck surgery. While there her care was wonderful and everyone was very kind. The therapy department was exceptional. My mom could not even sit up on her own when she first arrived. Through the hard work of the OT and PT departments, she walked out the door and is able to completely care for herself. She is stronger now than before her surgery. All of those girls in therapy deserve a raise for their top notch skills, and ability to motivate, all while keeping it fun. A special thanks goes to Erin and Lori, in therapy. They went above and beyond to prepare mom for a safe return home. She loved working with them.

This was my first personal experience with a rehab facility and I came there with much trepidation and a little bit of fear. I had broken my leg and was totally dependent on the staff at this facility. Top to bottom this facility was awesome!!! Every interaction was kind, caring and helpful! I can’t say enough about my care. I know this sounds like a fake review but, it is not. Such a positive experience. The staff here truly do personally care for every patient they encounter.I want to thank each and every person I encountered there for helping me and having faith that I could be successful in my rehab and be able to go home.So very thankful for all the staff at Buckeye Care &Rehabilitation.

Spent approximately 5-1/2 months for rehab from very severe spinal inflammation. When I first arrived my pain levels from nerve damage were extremely high. With their very qualified nursing staff the helped to build a well working pain management routine. The Physical Therapy team are sovond .

My brother left vanceburg rehab rehabilitation where he had contacted Covid then he was transported to SOMC from there he was transported to buckeye rehab rehabilitation. My brother has chronic lung disease and heart disease. The first day after being admitted his evening nurse was positive with bronchitis. I know this because his nurse told me he had bronchitis. I kept hearing someone cough and sneeze in the background and he said that was him so I rushed up there that evening to get my brother before they possibly Kill him. I think they should be investigated. Beware this place is not a place for your loved one.

My father was in here for 30 days after a stroke. We had POA in place and Cassie did not follow it. Upon discharge my sister was there to gather his belongings and the nurses aides were in the hallway talking about him and saying negative things regarding his ability to do things. They treated him poorly the whole day. They did not clean him up left feces on his bottom and threw on another diaper.( have evidence of this as well) The therapy they provided is not beneficial for stroke patients. I have recordings where my father only received 10-15 mintues top of actual therapy. Do not send your loved ones here they are all about profit and nothing else. Look up this place do your research.

This place was the main factor that caused my husband to die. Total neglect, everytime I went to visit him he was soaking wet, he had a massive stroke and had no use of his left side. An aide sat him on the side of the bed left him he fell forward hitting his eye on the wheel chair. They didn’t send him to the er, he was a kidney transplant patient and they weren’t giving his anti rejection meds like they should’ve been. His blood sugars were really high. He drank a lot of water & his water jug was always bone dry. I complained multiple times called state of course date didn’t find anything wrong because they’re just as incompetent. They just walk in look at the place and leave. They probably should assess every resident out there. My husband ended up getting.ESBL and since he is immunocompromised, it was too hard for him to fight he was in and out of the hospital moved him to another facility in Canal. They neglected him too. I used to work for a nursing home many years ago and I just don’t get how people can neglect these patients and treat them as horrible as they do. My husband later died on Memorial Day if I could’ve quit my job and took care of him at home I would’ve

My Dad’s care was wonderful here! The entire staff from admissions to social services to nursing, all deserve to be recognized! Josh has helped in tremendous ways handle insurance adjustments to take a burden off me while going through this trying time. Cassie is amazing at what she does and I am so grateful for all her help! So many great nurses, especially Michelle and Tammy. Thank you all so very much for all you do!

I am so impressed with the care they gave to my 89 year old mother after her hip surgery. They worked diligently with her in physical therapy and the aids and nurses were very caring and attentive. They were quick to answer our questions and respond to our concerns. I was also thrilled to see so many people working there.

During my stay at this facility the food was amazing, the people who work here are amazing and they are always willing to help you, the activities department has a lot of fun activities to do during the day for all the residents. I would highly recommend this facility.

I would like to commend the following for being highly motivating; professionalism; and always greeting me with a smile. Showing genuine empathy and compassion that the nursing field can be proud of. Especially Martha, Sara, Randall, Jessica, Ericka nursing assistant exceptionally and highly motivated and therapists Lori and Kathleen. Treatment nurse Debbie removed catheter with minimal amount of discomfort. Thank you for a pleasant stay.

I thought my mom received excellent care at this facility, but when she passed away there, I knew it! Staff were loving, caring and went above and beyond to make sure we had whatever we needed. I spent 6 days there with her at that time.
